Readmissions
The Texas Center for Quality & Patient Safety has developed an iniative to reduce hospital readmissions occurring within 30 days of discharge. While reasons for hospital readmissions differ across hospital settings and facility types, this project will include a cohort of rural hospitals.Project RED (Re-engineered discharge) is an evidence-based framework for reducing preventable readmissions. The intent of this program is to implement an evidence-based protocol for reducing readmissions and to work with hospitals to form a project team responsible for implementation. Participants will have the opportunity to collaborate with other facilities (urban and rural) across the state to share effective process changes and to develop and test novel approaches for success in reducing readmissions. Interested hospitals must commit to participating for 18 months, submit data on a monthly basis, and participate in learning sessions via telephone conference. Participation is free for THA members. Hospital executives and quality leaders are encouraged to participate and share their facility's unique needs.
